I agree pictures of my child may be used by Northern Arts Factory; this may include our web site, social networking sites and other publicity. (Please notify the team if you do not want this to happen)
I will NOT post any video footage recorded at Northern Arts Factory on the internet.
I must inform Northern Arts Factory staff if someone else will be collecting my children.
I agree that Northern Arts Factory may use my contact details to inform me of future events.
I agree to pay fees in half termly blocks and payments must be made by the first week of each term.
